The Cody Dieruf Benefit Foundation (CDBF) is excited to announce brand new applications for the Medical Assistance Program and the REC & FIT Scholarship. These new applications can be completed in nearly half the time of their predecessors, without compromising on quality or information collection.
Both applications have been given a unifying design to create a feeling of consistency and familiarity. In fact, both applications even share an identical first page to collect simple personal and contact information. If an applicant has completed a REC & FIT Scholarship, for example, then the Medical Assistance Program application will feel familiar.
The Medical Assistance Program application has been divided from one eight-page application to three two-page applications. This creates a quicker, simpler application process for our constituents. The three new applications now focus on medical travel assistance, out-of-pocket expense reimbursement, and special life experiences individually.
The greatest change CDBF has implemented is the availability of online applications. This brings the REC & FIT Scholarship and Medical Assistance Program inline with our Mother’s Retreat, which was already available for online registration.
The new online application allows for an extremely quick application process. On browsers that support the feature, applicants can autofill a majority of the information necessary to apply.
The online application process also allows for multiple conveniences such as easily uploading of images and documents and the ability to save a partially completed application to be finished later.
With our website’s new Facebook Chat bubble, applicants can easily ask questions while they’re completing their application.
Online applications are submitted instantly to CDBF, making them a much quicker alternative to mailing-in a paper application.
Applying to a CDBF program has never been easier. Our main menu, at the top of this website, has been reorganized to include a bright blue APPLY button for quick access to our programs.
We hope this simpler and friendlier application process will inspire new families to take the plunge and join our CDBF family.
